2. Identify the program elements of hybrid cardiac rehabilitation
3. State the evolution of hybrid cardiac rehabilitation billing and reimbursement
4. Recall data collection for hybrid cardiac rehabilitation program assessments and outcomes

Chronic Disease Risk Factor Management
At the conclusion, participants should be able to:
1. Recognize effective blood pressure management strategies while using telemedicine
2. Identify diabetes management strategies by using telemedicine
3. Identify cholesterol management strategies by using telemedicine
4. Explore management strategies for additional cardiac risk factors by using telemedicine
