Description
This module will provide an overview of health and wellness coaching, the role a health and wellness coach can play as part of a care team and how health care professionals can use coaching skills to increase self-efficacy with their patients. The health professional will learn about the difference between the expert and coach conversational approaches, and how to move a patient through the coaching process by uncovering a patients values, and developing SMART goals and action plans related to their health goals.

Learning Objectives
At the conclusion, participants should be able to:

1. Define the role and responsibilities of a health coach
2. Describe the coaching process and arc to encourage a patient to move through the behavior change process
3. Distinguish the difference between the coach and expert approaches
4. Identify the role a coach can play as part of a care team
5. Identify how to design a patient-centered health vision
6. Describe values and gal-focused coaching

Successful completion of this CE activity includes the following:

2. Complete the posttest with a score of 80% or higher
3. Complete the course evaluation
